Use this form for each of the competences.

Employer Proposer (Form 301)

Pass this on to your employer to complete.

IHIE CPD (Forms B01, B02, B03, B04)

Profile of competence and needs (B01), Annual professional development plan (B02), Three year plan (B03), Professional development record (B04).

Forms needed to apply

You will also need to fill in a Transfer Form if you are an existing Member or Affiliate or an Application Form to join if you are not. You will also need to complete the Subscription Payment Form to send with your fees.
